The Iowa Incident and Iowa Arrest Reports were developed by the Iowa Department of Public Safety (DPS) as part of the transition from summary-based uniform crime reporting to incident-based uniform crime reporting. Most law enforcement agencies in the State of Iowa use these forms to gather the information, which is entered into the various databases maintained by DPS, including the database from which IBUCR reports are derived. These reports, shown below, are the forms utilized by the Atlantic, Oskaloosa, and Norwalk Police Departments.
Some agencies add to the basic forms certain information that serves to identify the specific agency. Another report available below displays the Iowa Incident Report utilized by the Pella Police Department, which has added their ORI number to the field in which that data are entered. The Iowa Incident Report and Iowa Arrest Report utilized by the Harrison County Sheriffs Office are also provided. This agency has added the agency name as well as the ORI number to both reports. This agency has also replaced page 2 of the arrest report to gather additional information as well as provide space for a narrative. On the Iowa Incident Report, the agency has added a third page to gather additional information and provide additional narrative space.
Some law enforcement agencies develop their own forms through which IBUCR data are gathered, such as the Incident Report and Arrest Report utilized by the Altoona Police Department. Also available is the Incident Report and adult Arrest Report utilized by the Keokuk Police Department.
